Sign this form and return it with your first payment of $100.00 by November 15th to reserve your child’s place on this musical journey!!

Total Cost of trip per student: approx. $500.00 (plus some food on the trip)
A final cost will be determined once we know how many students plan on attending. Additionally, your cost could go down if you fundraise! **The poinsettia fundraiser just started!!

PAYMENT: The initial deposit will be $100; it is due November 15th and is not refundable. After that, minimum payments of $125 will be due Jan 15 and Feb 15 with balance due Mar 15. Please have your student place payments in the Middle School Band Box.
